It is important that we have an emergency contact if a parent can't be reached.





Permissions
I hereby give permission to the staff of the Boston Workers Circle to administer basic first aid and/or CPR to my teen and/or take my teen, to a hospital for medical treatment when I cannot be reached or when delay would be dangerous to my teen's health. I agree to pay for any expenses incurred. 


I/we hereby grant permission to Boston Workers Circle to use photos or video of my child(ren) and/or me/us, in printed or electronic materials (including our member newsletter, website, facebook/instagram, and live streams). 

When photos are posted and/or used, no names or other identifying information is included. With this in mind, we ask that all families opt-in at this time.  

If you opt out, we require a recent photograph of your child (emailed to meira@circleboston.org) AND ask that you discuss your decision with your child so they understand that they will be excluded from all photos at TASC to avoid accidental posting and/or use of their photo in the future.

Payment
Choose the appropriate tuition rate for your family below. Cost should never be a barrier to accessing BWC programs. Email info@circleboston.org if you'd like a payment plan or tuition assistance.  

BWC Membership is required for TASC families: If you haven't already renewed for 2026-27, please renew membership after completing this form.
